Patients → Forms to send and track them. Settings → Patient Forms to build the templates. Available when the FORMS feature is enabled. Forms replace the clipboard. You send a link by text or email, the patient fills it in on their phone before the visit, and the answers come back attached to their record.

Building templates

Typical templates: registration and demographics, medical history, consent, insurance details, and practice-specific policies. Keep them short. A form is filled in on a phone, often in a car park. The completion rate falls off a cliff past a couple of screens. Split a long form into two rather than making one long one.

Sending a form

Send it from the patient’s record, or as part of the flow around a new appointment. The patient gets a link; opening it does not require an account or a password.

Status

Opened is the status worth watching. A patient who opened and did not finish hit something: a question they could not answer, or a form that was too long.

When to send

At booking

Highest completion rate. The patient is already thinking about the appointment.

With the reminder

Catches the ones who ignored the first send, while there is still time to act on the answers.
Sending on the day of the visit gets you a form filled in the waiting room, which is where you started.
